What is the difference between Deep Plane Facelift and SMAS facelift?

The main difference between deep plane facelift and SMAS facelift is the location of the SMAS dissection. The deep plane is the potential space that exists between this SMAS layer and the layer of the face containing the muscles of facial expression. Deep plane facelift procedures involve dissection in this plane, or space.

All effective facelift procedures address the SMAS tissues to some degree. Although, traditional SMAS facelift address the SMAS tissues more laterally (or closer to the ear) and more superficially (closer to the skin surface). In contrast, deep plane facelift allows for treatment of the deeper tissues more medially, near the area of greatest concern, and allows for additional improvement in the cheeks, neck, and midface. Additionally, deep plane facelift frees and mobilizes the SMAS from four separate retaining ligaments.

How is Deep Plane Facelift Surgery Performed?

Deep plane facelift is performed with general anesthesia. Incisions are hidden within the hairline and behind the ear. A small amount of skin is elevated from the underlying tissues around the ears. The SMAS layer is entered at a point between the lateral canthus (point where the upper and lower eyelid meet) and the angle of the mandible (jaw). The deep plane tissues, located just deep (or beneath) the SMAS fascia are carefully dissected toward the nasolabial fold, corner of the mouth, and jowl area.

By separating the 4 key facial retaining ligaments, the tissues of the deep plane are carefully separated and freed from the layer containing the muscles of facial expression. The freely mobile SMAS tissues and overlying (attached) skin are carefully repositioned to a more youthful, rejuvenated position. The skin is closed in a tension-free fashion which allows for optimum healing. A soft cotton head wrap dressing is placed.

To achieve the best results, facelift may be performed in conjunction with other neck and jawline rejuvenation procedures such as chin augmentation, chin implant and jaw augmentation, neck liposuction and neck lift.
